Service areas
Skilled Nursing
include post-op care and teaching, care evaluation and coordination, wound, ostomy, and catheter care, disease and medication management and teaching, bladder and bowel training, and Antibiotics/TPN/IV Therapy.
Physical Therapy
Our therapists will assist patients with therapeutic exercises, balance restoration, prosthetic training, gait and transfer training, post-op teaching and rehabilitation, and maintenance therapy.
Speech Therapy
Our speech-language pathologists will help patients improve aphasia (impairment of speech), improve dysphagia (swallowing problems), and provide management and training for cognitive disorders.
Occupational Therapy
This therapy will provide patients with sensory integration, improving fine motor skills, energy conservation, and training in daily living activities.
Medical Social Worker
Patients can get counseling for long-term health care needs, assessment of social and emotional wellbeing, long-term placement assistance, and community resource identification and planning.
Home Health Aide
Our aides will assist patients with various daily living tasks until they recover and regain independence. This service may include grooming, personal care, and therapy-driven exercises.
Psychiatric Nursing
Patients can receive services that are tailored for their medical, psychological, and behavioral care needs. registered nurses with specialized training that help assess, address, and monitor mental health and behavioral conditions. FMS has nurses with this training on its staff.
Additional Services
Patients can depend on us for additional care that includes transitional care, rehospitalization risk assessment, community living support, community access, nutrition evaluations, as well as coordinating with infusion therapy and medical home equipment providers.
