Faith Medical Home Health is a Medicare-certified home health agency serving the residents of DeKalb, Cobb, and Clayton counties with exceptional care. While our home health division is newly established, our journey in healthcare began much earlier. Since opening our doors in 2002, we have been dedicated to providing quality support services to diverse communities, including seniors, youths, and individuals with disabilities.
Our roots trace back to over two decades of commitment and service. Founded on April 18, 2001, our parent company started its operations on January 1, 2002, with a mission to offer compassionate and timely care. Over the years, we have extended our reach, delivering support in hospitals, vocational rehabilitation centers, day centers, and private homes. Our comprehensive services have been tailored to meet the unique needs of each client, ensuring that everyone receives the dignity and respect they deserve.

FMS’s administration recognizes the importance of continually monitoring relevant aspects of care provided to patients. FMS’s quality improvement program maintains oversight and development of quality indicators that have been proven to measure aspects of care consistently. On a monthly and quarterly basis, staff will evaluate specific quality indicators. Results of the evaluation process will be summarized and reported to leadership to ensure that FMS’s patients receive high quality, effective care.
